The Coachella Valley is a unique healthcare community, and having lived and worked locally for many years, we understand its providers, resources, and care networks in a way that only true local experience can provide. We maintain extensive knowledge of in-home caregiving agencies — including both W-2 and independent contractor (1099) models — as well as skilled nursing facilities, assisted living and memory care communities, hospice and palliative care providers. Through years of building professional relationships and firsthand experience, we have developed reliable contacts and practical insight that allow us to guide families quickly and confidently to the right resources. This local expertise, combined with compassionate advocacy and healthcare leadership, makes us a trusted source of guidance for families navigating care in the Coachella Valley.

🌿 About Us – Our Story

Robert Ellis Case Management was founded from personal experience, deep compassion, and a desire to make healthcare easier to navigate for families. When my father became ill at just 55 years old, even with many years of experience in the healthcare industry, I found myself feeling confused, frustrated, and overwhelmed while trying to secure the best possible care for him. Experiencing the system firsthand revealed just how complex, fragmented, and difficult healthcare truly is — even for those who work within it every day.

Through that journey, I learned the true ins and outs of the healthcare system and how critical strong advocacy is for patients and families. I saw firsthand that advocating for yourself, for your loved one, or for your patient can make all the difference in outcomes, clarity, and peace of mind. That experience became the foundation for what would later become Robert Ellis Case Management.

Walking through my father’s illness and ultimately his passing changed the way I viewed healthcare and my role within it. He believed deeply in the importance of family, community, personal honor, and keeping one’s word — values that guided how he lived and how he treated others. I wanted to transform that experience into something meaningful — something that honored not only who he was, but how he lived his life, grounded in love, compassion, empathy, and integrity.

Robert Ellis Case Management was born out of love for my father, Robert Ellis Black, and a commitment to carry his legacy forward by serving families with honesty, respect, accountability, and unwavering advocacy.

Today, we exist to stand beside individuals and families as a trusted guide, helping them navigate care decisions with clarity, dignity, and confidence during some of life’s most challenging moments.

Complimentary Discovery Call

🌿Our complimentary 15-minute discovery call is an opportunity for you to share your situation, ask questions, and learn how Robert Ellis Case Management & Advocacy can support you or your loved one. During this brief call, we will discuss your immediate needs, determine whether our services are the right fit, and outline potential next steps. There is no obligation — this call simply helps you gain clarity and confidence about your options.

Initial Assessment & Care Plan

🌿 Following the discovery call, we offer a comprehensive initial assessment designed to fully understand the client’s medical history, current care needs, support system, living environment, and personal goals. This assessment may include record review, provider coordination, medication overview, safety evaluation, and conversations with family members or caregivers.

Using this information, we develop a personalized care plan that outlines clear recommendations, priorities, and next steps. The care plan serves as a roadmap for coordinating services, improving communication among providers, identifying appropriate resources, and advocating for the client’s best interests. Our goal is to create clarity, reduce stress, and ensure each client receives thoughtful, well-coordinated care tailored to their unique situation.

Professional Care Management Plans

🌿 Essential Support Package $750 month

Best for: Clients who need light guidance, oversight, and periodic advocacy.

Includes:

Up to 5 hours per month of care coordination and advocacy
Monthly care plan review and updates
Provider communication and appointment coordination
Resource referrals and recommendations
Secure documentation and care tracking
Email and limited phone support

🌿Hourly Care Management & Advocacy Services

Monday–Friday, (8:00am–5:00pm)

$105 per hour

Services may include:

Care coordination and provider communication
Medical appointment support and follow-up
Resource research and referrals
Care plan updates and documentation
Family communication and advocacy
Placement guidance and transitions of care
Insurance and benefit navigation
